JOB TITLE: Clinical Manager                                                                                    REPORTS TO: Director of Nursing (DON)
LOCATION: Pittsburgh, Pennsylvania

A. BASIC PURPOSE
The Clinical Manager provides oversight of all patient care services and nursing personnel for the Pittsburgh branch location. This position is responsible for supervising nursing operations, coordinating patient care, ensuring regulatory compliance, supporting field clinicians, and maintaining high standards of quality care.
Responsibilities include making patient and personnel assignments, coordinating care delivery, ensuring timely completion of documentation, supporting staff development, and assuring that patient needs are continually assessed. The Clinical Manager also assists in the development, implementation, and updating of individualized plans of care.
This role may require maintaining an active clinical caseload while simultaneously managing and supporting other nurses based on agency census and operational needs.

B. PRIMARY RESPONSIBILITIES
Planning, Organizing & Implementation

  1. Provides direction, supervision, consultation, and support to nursing staff.
  2. Oversees clinical operations for the Pittsburgh branch to ensure compliance with state, federal, payer, and agency standards.
  3. Coordinates patient care services, staffing assignments, and nursing schedules.
  4. Assures patient needs are continually assessed and addressed timely.
  5. Ensures development, implementation, and updates of individualized plans of care.
  6. Reviews and monitors timely completion of all clinical documentation including:
    • Admissions / Start of Care
    • Recertifications
    • Supervisory visits
    • Resumptions of Care
    • Discharges
    • Readmissions
    • Visit notes
    • Physician orders and care plans
  7. Performs skilled nursing visits and home visits as needed.
  8. May maintain a personal caseload while managing branch nursing operations.
  9. Serves as a clinical resource for nurses and field staff regarding patient care concerns, documentation, and compliance issues.
  10. Works collaboratively with intake, scheduling, HR, EVV, and administrative staff to ensure smooth operations.
  11. Participates in audits, quality assurance reviews, and performance improvement initiatives.
  12. Maintains accurate clinical records and ensures documentation standards are met.
  13. Supports infection control, patient safety, and risk management initiatives.
  14. Performs other duties as assigned.
Human Resource Management
  1. Provides orientation, mentorship, coaching, and performance feedback to clinicians.
  2. Conducts performance evaluations during probationary periods and annually thereafter.
  3. Identifies staff development needs and supports continuing education.
  4. Addresses performance concerns promptly and professionally in coordination with the DON.
  5. Helps maintain adequate staffing levels to meet patient census demands.
Fiscal / Operational Management
  1. Promotes efficient utilization of nursing staff and scheduling resources.
  2. Assists in balancing staffing needs with census growth and patient demand.
  3. Supports productivity expectations and timely visit completion.
  4. Coordinates branch readiness for referrals, admissions, and ongoing patient service needs.
  5. Assists in resolving patient, family, and customer service concerns.
  6. Leadership / Professionalism
  7. Maintains current knowledge of home health regulations, standards, and best practices.
  8. Demonstrates leadership, professionalism, and sound clinical judgment.
  9. Effectively prioritizes competing demands and manages multiple responsibilities.
  10. Maintains confidentiality of patient and agency information in accordance with HIPAA.
  11. Promotes a positive, accountable, and team-oriented work environment.
  12. Serves as a role model for communication, attendance, dependability, and professionalism.

  1. Maintains a professional attitude and communicates effectively with patients, families, staff, physicians, referral sources, and community partners.
  2. Demonstrates optimism, teamwork, and commitment to patient-centered care.

D. EDUCATION AND EXPERIENCE
  1. Graduate of an accredited nursing program.
  2. Active and unrestricted Pennsylvania RN license required.
  3. Minimum two (2) years home health experience required.
  4. Prior supervisory or leadership experience preferred.
  5. Strong knowledge of OASIS, home health documentation, physician orders, and care planning.
  6. Valid driver’s license, insured vehicle, and ability to travel as needed.
  7. Satisfactory background clearances and references required.
