Overview
Ideal Home Health Agency is seeking an energetic, dynamic, and highly motivated Clinical Manager to lead and support our nursing operations at the Pittsburgh branch. The Clinical Manager will oversee all nursing-related matters for the Pittsburgh location, ensuring high-quality patient care, regulatory compliance, and smooth operational workflow.
Qualifications
- Registered Nurse (RN) with an active Pennsylvania license (required)
- BSN or higher degree strongly preferred
- Minimum 2–3 years of home health experience preferred
- Prior leadership or supervisory experience preferred
- Strong understanding of home health regulations, OASIS, POC development, and clinical documentation
- Excellent communication, organizational, and problem-solving skills
- Ability to lead with professionalism, integrity, and a growth mindset
Key Responsibilities
Clinical Leadership & Oversight
- Manage all nursing operations for the Pittsburgh location, ensuring compliance with state, federal, and agency standards.
- Provide clinical guidance, support, and mentorship to nursing staff.
- Ensure timely completion of all nursing documentation, including assessments, recertifications, supervisory visits, and care plans.
- Collaborate closely with the DON to address clinical issues, performance concerns, and workflow improvements.
- Perform home visits as needed for:
- Admissions (Start of Care)
- Recertification assessments
- Supervisory visits
- Discharge
- Readmission
- Resumption of care
- And so on.
- Take on and manage one skilled nursing client to maintain hands-on clinical involvement and ensure quality of care.
- Ensure timely submission and follow-up of MD orders, POCs, and recertification documents.
- Support scheduling, coordination, and communication with families, physicians, and interdisciplinary teams.
- Promote uniformity in operational procedures and uphold agency standards across all functions.
- Participate in audits, quality assurance reviews, and performance improvement initiatives.
- RN is also responsible for discharge, re admit or resumption of care. Support the nursing staff diligently to meet the company expectation of completing all documentation timely.
